If a breath of crisp winter air makes you say, “Ouch!” you're not alone. According to a study published in the Journal of the American Dental Association, one of every eight people suffers from tooth sensitivity. And for those individuals, winter can be a particularly challenging time of year.
Tooth sensitivity can result when the inner part of the tooth, called dentin, is exposed. Dentin is normally protected by enamel above the gum line and cementum below, but if the protective coating is lost, then temperature, pressure and acid from food and drinks can activate the nerves inside the tooth.
If you suffer from tooth sensitivity, these tips may help:
Avoid acidic foods and beverages. It may be common sense to stay away from foods and drinks that are hot or cold enough to make you wince, but also avoid those that are acidic, as acid can erode tooth enamel and increase tooth sensitivity.
Wait an hour to brush your teeth. After consuming acidic food or beverages, give your saliva time to neutralize the acid and strengthen the enamel surface to prevent erosion.
Brush gently. Gums can recede due to over-aggressive brushing, exposing sensitive tooth roots. So brush your teeth gently with a soft-bristled toothbrush and rinse with lukewarm water.
Use toothpaste for sensitive teeth. Toothpaste that is specially formulated for sensitive teeth blocks the pores in the tooth's surface where sensitivity can occur. It may also to help to rub the toothpaste on sensitive areas.
Sometimes, however, sensitive teeth result from dental problems that need professional treatment in the form of an at-home prescription, an in-office treatment like bonding or sealants, or a procedure like a gum graft or root canal. Accordingly, here's the most important tip of all:
Schedule a dental appointment. In an exam, we can look for the cause of your tooth sensitivity so it can be treated properly. Sensitivity may result from receding gums, tooth decay, erosion of the enamel, or other dental problems, such as the following:
- Tooth-grinding. If we detect signs of a nighttime tooth grinding habit that you may not even be aware of, we may recommend a nightguard to wear while sleeping.
- A root infection. If your tooth remains sensitive 30 seconds after eating or drinking something hot or cold, the pulp inside your tooth may be damaged. You may need root canal therapy to remove the infection and stop it from spreading.
- A cracked tooth. A crack in a tooth may not be visible due to its size or location, but a compromised tooth surface can cause sensitivity and could lead to bigger problems if not treated.

Tooth sensitivity can be disheartening: you’re always on your guard with what you eat or drink, and perhaps you’ve even given up on favorite foods or beverages.
The most common cause for this painful sensitivity is dentin exposure caused by receding gums. Dentin contains tiny open structures called tubules that transmit changes in temperature or pressure to the nerves in the pulp, which in turn signal pain to the brain. The enamel that covers the dentin, along with the gum tissues, creates a barrier between the environment and dentin to prevent it from becoming over-stimulated.
Due to such causes as aggressive over-brushing or periodontal (gum) disease, the gum tissues can recede from the teeth. This exposes portions of the dentin not covered by enamel to the effects of hot or cold. The result is an over-stimulation of the dentin when encountering normal environmental conditions.
So, what can be done to relieve painful tooth sensitivity? Here are 3 ways to stop or minimize the symptoms.
Change your brushing habits. As mentioned, brushing too hard and/or too often can contribute to gum recession. The whole purpose of brushing (and flossing) is to remove bacterial plaque that’s built up on tooth surfaces; a gentle action with a soft brush is sufficient. Anything more than two brushings a day is usually too much — you should also avoid brushing just after consuming acidic foods or liquids to give saliva time to neutralize acid and restore minerals to the enamel.
Include fluoride in your dental care. Fluoride has been proven to strengthen enamel. Be sure, then, to use toothpastes and other hygiene products that contain fluoride. With severe sensitivity you may also benefit from a fluoride varnish applied by a dentist to your teeth that not only strengthens enamel but also provides a barrier to exposed dentin.
Seek treatment for dental disease. Tooth sensitivity is often linked to tooth decay or periodontal (gum) disease. Treating dental disease may include plaque removal, gum surgery to restore receded gums, a filling to remove decay or root canal therapy when the decay gets to the tooth pulp. These treatments could all have an effect on reducing or ending your tooth sensitivity.

“We all scream for ice cream,” the saying goes. But what if eating ice cream — or any very cold or hot food — literally makes you want to scream because your teeth hurt so much?
What causes sensitivity in teeth?
Understanding the anatomy of a tooth helps explain what happens when a tooth becomes sensitive to heat and cold. A tooth is composed of three types of tissue: a hard outer shell of enamel, the body of the tooth composed of the dentin, and an interior tissue of the pulp.
Enamel: The enamel forms the outside of the crown, the part of the tooth you normally see. Made of densely packed crystals of calcium, it is resistant to wear. It is not living tissue, and does not contain nerves, but it is capable of transmitting temperature like hot and cold.
Dentin: Inside the tooth's crown and root is a living tissue called dentin, which is a porous structure similar to bone. It is composed of microscopic tubules containing living cells, which are encased in a hard substance made of calcium crystals.
Pulp: The living dentin transmits sensation through to the pulp, which is in the center of the tooth and contains the tooth's blood vessels and nerves.
A tooth's enamel normally protects the dentin from exposure to extremes of temperature and pressure. If you wear away the enamel and expose the dentin, it will pass sensation through to the nerves in the pulp more directly. The result can range from a twinge to an excruciating pain.
Sensitivity can be caused by:
- Overzealous tooth brushing resulting in enamel wear and consequently dentin exposure and wear.
- Enamel and dentin erosion by acids in the foods and beverages you eat and drink.
- Tooth decay — the most common cause of sensitivity. Decay destroys enamel and dentin inflaming and infecting the living tissues of the pulp, which become increasingly painful.
What can you do to make your teeth less sensitive?
- Use a soft bristle tooth brush, and brush the affected teeth gently to remove all bacterial plaque. We can advise you on safe and effective brushing techniques.
- Use toothpaste that contains fluoride. Fluoride strengthens tooth surfaces and makes them more resistant to sensitivity and decay.
- Ask us about professionally applied fluoride varnishes or filling materials that can cover and replace sensitive or lost tooth structure.
Of course, if the problem is caused by tooth decay, make an appointment with us to remove the decay and place a filling in the sensitive teeth.
In a healthy tooth, a coating of enamel protects the crown — the part above the gum line — and a layer of cementum protects the tooth root below the gum line. Enamel and cementum are inert (nonliving) substances that do not respond to stimuli such as heat or cold; however, dentin, the living tissue below them, does. Dentin contains numerous microscopic tubules that readily transmit stimuli toward the nerve-filled center of the tooth (pulp tissue). Loss of protective enamel or cementum leaves dentin exposed to all sorts of stimuli in the oral environment, which can trigger “dentinal hypersensitivity” — anything from a mild twinge to shooting pain.
Fortunately, there are many options for treating hypersensitivity. The key to selecting the most appropriate one(s) is determining the cause(s). Some of the more common reasons for sensitivity due to dentin exposure include:
- Enamel erosion caused by an “acid attack” related to external (extrinsic) causes — i.e., consumption of acidic beverages/food — or internal causes — i.e., regurgitation of stomach acids due to gastroesophageal reflux disease [GERD] or the eating disorder bulimia
- Using an overly abrasive brush or toothpaste, brushing incorrectly or too frequently, or brushing too soon after an “acid attack” — all of which can result in a loss of enamel
- Tooth decay (dental caries or cavities)
- Tooth fracture or chipping: tooth grinding (bruxism) is a common cause
- Worn fillings
- Gum recession, due to age or improper tooth brushing, that exposes the tooth root
- Gum disease, which can result in gum recession
Sensitivity can also occur following a procedure like treating a cavity. Normally it subsides within a couple of weeks or so but if it continues there may be another underlying cause.
